腹腔镜夹钳开窗设计对夹持稳定性影响研究

Effect of Fenestrations in Laparoscopic Grasper on the Clamping Stability

摘要: 在多功能摩擦磨损试验机上模拟微创手术中的组织夹持牵引操作,研究了钳头与猪肝脏组织间的摩擦学行为,并基于钳头与组织间的夹持应力分布数值模拟结果,探讨了不同的腹腔镜开窗钳头与肝脏组织的相互作用行为。结果表明:在接触系数相同的情况下,开窗数量的增加有助于增大滑动摩擦力;三正方形开窗钳头由于开窗轮廓最长,在与组织相互作用过程中,使组织在轮廓边缘产生了更多的变形,有利于嵌入组织,与肝脏组织之间产生的能量耗散及滑动摩擦力最大,夹持稳定性最好;对于相同类型的矩形开窗钳头,接触系数存在一个阈值,当接触系数小于该阈值时,随着接触系数的减小,摩擦力减小,夹持稳定性会降低;钳头与肝脏组织接触过程中,较大的接触应力集中在钳头端部边缘和开窗轮廓边缘与肝脏组织接触处。

Abstract: The frictional behaviors between the fenestrated jaws of laparoscopic grasper and the liver tissue are investigated by using a UMT-Ⅱseries multi-specimen biomedical micro-tribometer to simulate the surgical manipulation and traction. Based on the numerical simulation results of the clamping stress distribution between the jaws and the liver tissue, the interaction behaviors between the different fenestrated-jaws and the liver tissue are also discussed. The results show that under the same contact coefficient, increasing of the number of fenestrated holes can help to enhance the sliding friction. The jaw with three squares fenestrations has the longest contour line, and causes more deformation in the tissue at the edge of the fenestrations during the interaction with the liver tissue, which helps the tissue embed into the fenestration. Since this type of fenestrated jaw can produce the largest energy dissipation and the sliding friction, its holding stability is the best. For the same type of rectangular opening jaws, there exists a threshold value of contact coefficient. The stability of the grasper declines with decreasing contact coefficient when the contact coefficient is less than the threshold. In addition, in the contact process of the jaws and the liver tissues, the greater contact stress is concentrated at the edge of the two ends of jaw and the edge of the open window during the contact process between the jaws and the liver tissue.
